FNU immunization requirements: what is due, and when
FNU asks for measles, mumps, rubella and varicella immunity and freedom from communicable TB at admission, then adds hepatitis B and Tdap before Clinical Bound. FNU no longer mandates the COVID-19 vaccine, but many clinical sites still do. For clinicals, everything is documented on FNU's own Student Health Verification Form and uploaded to CastleBranch.

The FNU list in one table
The catalog asks students to document “vaccination or immunity” to rubella, measles, mumps and varicella, and freedom from tuberculosis in a communicable stage, then hepatitis B and Tdap before Clinical Bound. Sorted by item, it looks like this.

Two deadlines: admission and Clinical Bound
The first set is part of admission. Before Frontier Bound, you order the Frontier Bound Compliance package through CastleBranch, which covers a background check, proof of immunizations and current medical record information. For Winter 2027 admits that package is due October 14, 2026, and FNU warns that missing it may cancel your admission.
The second set comes with clinical compliance, the term before Clinical Bound. Clinical Credentialing emails you in Week 1 of that term and the items are due in Week 5, so hepatitis B and Tdap documentation must be ready by then. If you are still completing a hepatitis B series, ask your health provider how its timing fits that Week 5 date.

The FNU Student Health Verification Form
For clinicals, the catalog requires you to complete all sections of the FNU Student Health Verification Form, including vaccinations or titers per FNU's Health Form Guidelines and Checklist. “Non-university forms will not be accepted,” so a printout from your employer's occupational health office cannot stand in for it, however complete.
The form “cannot be older than 12 months at any time during the clinical practicum.” FNU suggests planning the 750 hours across three terms, and a practicum that runs longer than planned can outlast a form completed early in the compliance window. The health examination has the same one-year rule: it must be current before any Bound and kept current throughout the clinical experience.
The health form runs on a 12-month clock that lasts through the whole practicum. Note the date it lapses, not only the date it is due.
COVID-19, influenza and other site rules
FNU's catalog says it “no longer mandates the COVID-19 vaccine,” but many clinical sites still require proof of vaccination or an approved exemption, and each site decides whether to accept an exemption request. COVID-19 documentation in CastleBranch is optional at the Frontier Bound stage.
The same logic applies to anything a site adds. The catalog says clinical sites may have additional requirements that students must complete to rotate there, and site-specific requirements are at the student's own expense. Influenza vaccination, a particular TB test or mask fit testing may appear in a hospital's onboarding even though FNU's list does not name them.
Ask each site early. A hospital that requires COVID-19 vaccination and seldom grants exemptions is worth knowing about before you commit a CSAF to it, not after Clinical Credentialing has started work on the agreement.
What FNU's public pages leave out
Several details students search for are not in public FNU sources. The catalog's lists do not say which TB test FNU accepts (a skin test, a blood test or a chest X-ray), whether influenza vaccine is required, whether N95 fit testing is needed, or which titers satisfy each item. Those rules sit in the Health Form Guidelines and Checklist, which is not public.
Look for the checklist in CB101 in Canvas, on the Clinical Credentialing pages in Banyan Tree 2.0, or in your CastleBranch requirements. If the wording there is unclear, ask Clinical Credentialing directly rather than guessing; a rejected upload in Week 5 costs more time than a question in Week 1.

Questions FNU students ask
Does FNU require the COVID-19 vaccine?
No. The catalog says FNU no longer mandates the COVID-19 vaccine. Many clinical sites still require proof of vaccination or an approved exemption, and each site can accept or deny exemption requests. What TB test does FNU accept?
FNU's public sources do not say. The catalog asks for documentation of freedom from tuberculosis in a communicable stage, but the accepted test type sits in the Health Form Guidelines and Checklist, which needs an FNU login. Check CB101 or your CastleBranch requirements, and ask Clinical Credentialing if it is still unclear.
When do I need hepatitis B and Tdap for FNU?
Before Clinical Bound. The catalog lists hepatitis B and Tdap as items required prior to Clinical Bound, separate from the measles, mumps, rubella, varicella and TB documentation needed at admission. In practice they go into CastleBranch with your clinical compliance, due in Week 5 of the term before your Clinical Bound term.
Can I use my employer's health form instead of FNU's?
No. The catalog says you must complete all sections of the FNU Student Health Verification Form, and that non-university forms will not be accepted. Records from your employer, such as vaccination dates or titer results, can still be the evidence you transfer onto FNU's form.
Do I need titers, or will vaccination records do?
